Usually this service is arranged by the patient or case worker. It is recommended that arrangements or the order is made as soon as the appointment is made. Our service is door to door and our drivers are all trained to assist patients as necessary and get them to their appointment in a safe and timely manner. The driver will wait for the patient at the place of appointment and will be ready to take them back as soon as they are finished. Usually this service is arranged by the patient's case worker at CCS. It is recommended that arrangements or the order is made as soon as the appointment is known. Our service is door to door and our drivers are all trained to assist patients as necessary and get them to their appointment in a safe and timely manner. Since the patient is usually a minor, parents are encouraged to go with the child. Up to 4 passengers can be accommodated. Unless the appointment is out of town, when the appointment is over a call is made to our office and the driver then goes back to take them home. San Joaquin County Dept of AgingTransportation for MSSP and FCSP beneficiaries. Usually this service is arranged by the transportation coordinator at the county Social Services department. It is recommended that arrangements or the order is made as soon as the appointment is known. Unless the appointment is out of town, when the appointment is over a call is made to our office and the driver then goes back to take them home. Hospital Discharge to SNFTransportation for patients discharged from hospital to SNF for Medi-Cal (Medicaid) beneficiaries is most welcome.
